DebugInfo: Move DIScope::getName() and getContext() to MDScope

Continue gutting the `DIDescriptor` hierarchy.  In this case, move the
guts of `DIScope::getName()` and `DIScope::getContext()` to
`MDScope::getName()` and `MDScope::getScope()`.
