[dsymutil] Implement dSYM bundle creation

A dSYM bundle is a file hierarchy that looks slike this:
 <bundle name>.dSYM/
     Contents/
        Info.plist
        Resources/
           DWARF/
              <DWARF file(s)>

This is the default output mode of dsymutil.
