Incorporate various suggestions Chris gave during metadata review.

- i < getNumElements()  instead of getNumElements() > i
- Make setParent() private
- Fix use of resizeOperands
- Reset HasMetadata bit after removing all metadata attached to an instruction
- Efficient use of iterators
