floppy: do not corrupt bio.bi_flags when reading block 0
Commit
41a55b4de39 ("floppy: silence warning during disk test") caused
bio.bi_flags being overwritten, and its initialization to BIO_UPTODATE
in bio_init() to be lost.
This was unnoticed until
7b7b68bba5 ("floppy: bail out in open() if
drive is not responding to block0 read"), because the error value wasn't
checked for in the bio completion callback.
Now we are actually looking at the error, and the loss of BIO_UPTODATE
causes EIO to be wrongly passed to the callback, which confuses the
FD_OPEN_SHOULD_FAIL_BIT logic.
Fix this by not destroying previous value of bi_flags when setting
BIO_QUIET.

MIPS: ptrace: Avoid smp_processor_id() in preemptible code
ptrace_{get,set}_watch_regs access current_cpu_data to get the watch
register count/masks, which calls smp_processor_id(). However they are
run in preemptible context and therefore trigger warnings like so:

Since the watch register count/masks should be the same across all
CPUs, use boot_cpu_data instead. Note that this may need to change in
future should a heterogenous system be supported where the count/masks
are not the same across all CPUs (the current code is also incorrect
for this scenario - current_cpu_data here would not necessarily be
correct for the CPU that the target task will execute on).

m68k: Increase initial mapping to 8 or 16 MiB if possible
If the size of the first memory chunk is at least 8 or 16 MiB increase the
initial mapping to 8 resp. 16 MiB instead of 4 MiB.
This makes it possible to
1. Map more memory in the first node without running out of space for the
page tables,
2. Boot kernels that don't fit in 4 MiB (e.g. multi_defconfig).

m68k/mvme16x: Adopt common boot console
In a multi-platform kernel binary we only need one early console instance.
The difficulty here is that the common early console is started by
early_param(), whereas the MVME16x instance is started later by
config_mvme16x(). That means some interrupt setup must be done earlier.

m68k: Multi-platform EARLY_PRINTK
Make the boot console available to more m68k platforms by leveraging
the head.S debug console.
The boot console is enabled by the "earlyprintk" command line argument
which is how most other architectures do this.
This is a change of behaviour for the Mac but does not negatively impact
the common use-case which is not debugging.
This is also a change of behaviour for other platforms because it means
the serial port stays quiet when CONFIG_EARLY_PRINTK is not enabled. This
is also an improvement for the common use-case.

vfs: fix vmplice_to_user()

6130f5315ee8 "switch vmsplice_to_user() to copy_page_to_iter()" in
v3.15-rc1 broke vmsplice(2).
This patch fixes two bugs:
- count is not initialized to a proper value, which resulted in no data
being copied
- if rw_copy_check_uvector() returns negative then the iov might be leaked.

If we try to perform a kexec when the machine is in ST (Single-Threaded) mode
(ppc64_cpu --smt=off), the kexec operation doesn't succeed properly, and we

Instrumenting this piece of code revealed that the cpu_up() operation actually
fails with -EBUSY. Thus, only the primary threads of all the cores are online
during kexec, and hence this is a sure-shot receipe for disaster, as explained
in commit
e8e5c2155b (powerpc/kexec: Fix orphaned offline CPUs across kexec),
as well as in the comment above wake_offline_cpus().
It turns out that cpu_up() was returning -EBUSY because the variable
'cpu_hotplug_disabled' was set to 1; and this disabling of CPU hotplug was done
by migrate_to_reboot_cpu() inside kernel_kexec().
Now, migrate_to_reboot_cpu() was originally written with the assumption that
any further code will not need to perform CPU hotplug, since we are anyway in
the reboot path. However, kexec is clearly not such a case, since we depend on
onlining CPUs, atleast on powerpc.
So re-enable cpu-hotplug after returning from migrate_to_reboot_cpu() in the
kexec path, to fix this regression in kexec on powerpc.
Also, wrap the cpu_up() in powerpc kexec code within a WARN_ON(), so that we
can catch such issues more easily in the future.

The assembler maintainer says:
I changed the ABI, something that had to be done but unfortunately
happens to break the booke kernel code. When building up a 64-bit
value with lis, ori, shl, oris, ori or similar sequences, you now
should use @high and @higha in place of @h and @ha. @h and @ha
(and their associated relocs R_PPC64_ADDR16_HI and R_PPC64_ADDR16_HA)
now report overflow if the value is out of 32-bit signed range.
ie. @h and @ha assume you're building a 32-bit value. This is needed
to report out-of-range -mcmodel=medium toc pointer offsets in @toc@h
and @toc@ha expressions, and for consistency I did the same for all
other @h and @ha relocs.
Replacing @h with @high in one strategic location fixes the relocation
errors. This has to be done conditionally since the assembler either
supports @h or @high but not both.

blk-mq: allow non-softirq completions
Right now we export two ways of completing a request:
1) blk_mq_complete_request(). This uses an IPI (if needed) and
completes through q->softirq_done_fn(). It also works with
timeouts.
2) blk_mq_end_io(). This completes inline, and ignores any timeout
state of the request.
Let blk_mq_complete_request() handle non-softirq_done_fn completions
as well, by just completing inline. If a driver has enough completion
ports to place completions correctly, it need not define a
mq_ops->complete() and we can avoid an indirect function call by
doing the completion inline.

PCI: Fix return value from pci_user_{read,write}_config_*()
The PCI user-space config accessors pci_user_{read,write}_config_*() return
negative error numbers, which were introduced by commit
34e3207205ef
("PCI: handle positive error codes"). That patch converted all positive
error numbers from platform-specific PCI config accessors to -EINVAL, which
means the callers don't know anything about the specific cause of the
failure.
The patch fixes the issue by converting the positive PCIBIOS_* error values
to generic negative error numbers with pcibios_err_to_errno().

PCI: Turn pcibios_penalize_isa_irq() into a weak function
pcibios_penalize_isa_irq() is only implemented by x86 now, and legacy ISA
is not used by some architectures. Make pcibios_penalize_isa_irq() a
__weak function to simplify the code. This removes the need for new
platforms to add stub implementations of pcibios_penalize_isa_irq().

PCI: Test for std config alias when testing extended config space
When a PCI-to-PCIe bridge is stacked on a PCIe-to-PCI bridge, we can have
PCIe endpoints masked by a conventional PCI bus. This makes the extended
config space of the PCIe endpoint inaccessible. The PCIe-to-PCI bridge is
supposed to handle any type 1 configuration transactions where the extended
config offset bits are non-zero as an Unsupported Request rather than
forward it to the secondary interface. As noted here, there are a couple
known offenders to this rule. These bridges drop the extended offset bits,
resulting in the conventional config space being aliased many times across
the extended config space. For Intel NICs, this alias often seems to
expose a bogus SR-IOV cap.
Stacking bridges may seem like an uncommon scenario, but note that any
conventional PCI slot in a modern PC is already the secondary interface of
an onboard PCIe-to-PCI bridge. The user need only add a PCI-to-PCIe
adapter and PCIe device to encounter this problem.

blk-mq: pass in suggested NUMA node to ->alloc_hctx()
Drivers currently have to figure this out on their own, and they
are missing information to do it properly. The ones that did
attempt to do it, do it wrong.
So just pass in the suggested node directly to the alloc
function.

Notify a PCI device driver when its device's access is about to be disabled
for an impending reset attempt, then after the attempt completes and device
access is restored. The notification is via the pci_error_handlers

dm mpath: really fix lockdep warning
lockdep complains about a circular locking. And indeed, we need to
release the lock before calling dm_table_run_md_queue_async().
As such, commit
4cdd2ad ("dm mpath: fix lock order inconsistency in
multipath_ioctl") must also be reverted in addition to fixing the
lock order in the other dm_table_run_md_queue_async() callers.

virtio_blk: fix race between start and stop queue
When there isn't enough vring descriptor for adding to vq,
blk-mq will be put as stopped state until some of pending
descriptors are completed & freed.
Unfortunately, the vq's interrupt may come just before
blk-mq's BLK_MQ_S_STOPPED flag is set, so the blk-mq will
still be kept as stopped even though lots of descriptors
are completed and freed in the interrupt handler. The worst
case is that all pending descriptors are freed in the
interrupt handler, and the queue is kept as stopped forever.
This patch fixes the problem by starting/stopping blk-mq
with holding vq_lock.

dm cache: always split discards on cache block boundaries
The DM cache target cannot cope with discards that span multiple cache
blocks, so each discard bio that spans more than one cache block must
get split by the DM core.

gpio: dwapb: use a second irq chip
Right new have one irq chip running always in level mode. It would nicer
to have two irq chips where one is handling level type interrupts and
the other one is doing edge interrupts. So we can have at runtime two users
where one is using edge and the other level.

gpio: mcp23s08: fixed count variable for devicetree probing
Fixed missing increase of count variable for devicetree path in driver
probing.
The gpio-mcp23s08 driver has two paths for getting the platform
registration information. One for the classic platform initialization
and one for openfirmware devicetree based initialization. The devicetree
based path is missing the increase of the count variable, which results
in the count variable to become negative in the later use, where it is
decreased. The count variable is used as an index into a vector. This
results in accessing invalid memory space and can result in an exception.
Tested this with an AM3352 SoC with two mcp23s17 on two chip selects as
well as on a shared chip select.

x86/xen: map foreign pfns for autotranslated guests
When running as a dom0 in PVH mode, foreign pfns that are accessed
must be added to our p2m which is managed by xen. This is done via
XENMEM_add_to_physmap_range hypercall. This is needed for toolstack
building guests and mapping guest memory, xentrace mapping xen pages,
etc.

drm/i915: Prevent negative relocation deltas from wrapping
This is pure evil. Userspace, I'm looking at you SNA, repacks batch
buffers on the fly after generation as they are being passed to the
kernel for execution. These batches also contain self-referenced
relocations as a single buffer encompasses the state commands, kernels,
vertices and sampler. During generation the buffers are placed at known
offsets within the full batch, and then the relocation deltas (as passed
to the kernel) are tweaked as the batch is repacked into a smaller buffer.
This means that userspace is passing negative relocations deltas, which
subsequently wrap to large values if the batch is at a low address. The
GPU hangs when it then tries to use the large value as a base for its
address offsets, rather than wrapping back to the real value (as one
would hope). As the GPU uses positive offsets from the base, we can
treat the relocation address as the minimum address read by the GPU.
For the upper bound, we trust that userspace will not read beyond the
end of the buffer.
So, how do we fix negative relocations from wrapping? We can either
check that every relocation looks valid when we write it, and then
position each object such that we prevent the offset wraparound, or we
just special-case the self-referential behaviour of SNA and force all
batches to be above 256k. Daniel prefers the latter approach.
This fixes a GPU hang when it tries to use an address (relocation +
offset) greater than the GTT size. The issue would occur quite easily
with full-ppgtt as each fd gets its own VM space, so low offsets would
often be handed out. However, with the rearrangement of the low GTT due
to capturing the BIOS framebuffer, it is already affecting kernels 3.15
onwards. I think only IVB+ is susceptible to this bug, but the workaround
should only kick in rarely, so it seems sensible to always apply it.
v3: Use a bias for batch buffers to prevent small negative delta relocations
from wrapping.

drm/i915: Only copy back the modified fields to userspace from execbuffer
We only want to modifiy a single field in the userspace view of the
execbuffer command buffer, so explicitly change that rather than copy
everything back again.
This serves two purposes:
1. The single fields are much cheaper to copy (constant size so the
copy uses special case code) and much smaller than the whole array.
2. We modify the array for internal use that need to be masked from
the user.
Note: We need this backported since without it the next bugfix will
blow up when userspace recycles batchbuffers and relocations.

drm/i915: Fix dynamic allocation of physical handles
A single object may be referenced by multiple registers fundamentally
breaking the static allotment of ids in the current design. When the
object is used the second time, the physical address of the first
assignment is relinquished and a second one granted. However, the
hardware is still reading (and possibly writing) to the old physical
address now returned to the system. Eventually hilarity will ensue, but
in the short term, it just means that cursors are broken when using more
than one pipe.

Input: synaptics - change min/max quirk table to pnp-id matching
Most of the affected models share pnp-ids for the touchpad. So switching
to pnp-ids give us 2 advantages:
1) It shrinks the quirk list
2) It will lower the new quirk addition frequency, ie the recently added W540
quirk would not have been necessary since it uses the same LEN0034 pnp ids
as other models already added before it
As an added bonus it actually puts the quirk on the actual psmouse, rather
then on the machine, which is technically more correct.

m68k: Toward platform agnostic framebuffer debug logging
Code subject to #ifdef CONSOLE is made more generic, as was apparently
intended by the original author.
Remove console_put_stats() routine. If it should be somehow useful, it
should also be useful on platforms without framebuffer debug logging. The
present implementation is only built #if defined CONFIG_MAC && defined
CONSOLE even though puts() works everywhere.

m68k/atari - atafb: convert allocation of fb ram to new interface
The new atari_stram_alloc interface returns kernel virtual addresses
even if the kernel runs in FastRAM. These addresses are not
guaranteed to be identical with the physical addresses. Since ST-RAM
mappings have not been set up by mem_init, virt_to_phys() and its
cousin do not work and the atari_stram_to_phys() etc. helpers must
be used to determine physical addresses.
fb.fix->smem_start needs physical addresses, fb.par->screen_base
needs virtual addresses. Take care of the virt-to-phys conversion
both on fb init and par changes.

m68k/atari - stram: alloc ST-RAM pool even if kernel not in ST-RAM
With the kernel loaded to FastRAM (TT-RAM), none of the ST-RAM
address range is mapped by init_mem, and ST-RAM is not accessible
through the normal allocation pathways as a result.
Implement ST-RAM pool allocation to be based on physical addresses
always (it already was when the kernel was loaded in ST-RAM).
Return kernel virtual addresses as per normal.
The current test for the kernel residing in ST-RAM always returns
true. Use the bootinfo memory chunk order instead - with the kernel
in FastRAM, ST-RAM (phys. 0x0) is not the first chunk.
In case the kernel is running from FastRAM, delay mapping of ST-RAM
pool until after mem_init.
Provide helper functions for those users of ST-RAM that need
to be aware of the backing physical addresses.
Kudos to Geert for his hints on getting this started.

ARM: vexpress: refine dependencies for new code
The versatile express changes for 3.16 introduced a number of
build regressions for randconfig kernels by not tracking dependencies
between the components right.
This patch tries to rectify that:
* the mach-vexpress code cannot link without the syscfg driver,
which in turn needs MFD_VEXPRESS_SYSREG
* various drivers call devm_regmap_init_vexpress_config(), which
has to be exported so it can be used by loadable modules
* the configuration bus uses OF DT helper functions that are not
available to platforms disable CONFIG_OF
* The sysreg driver exports GPIOs through gpiolib, which can
be disabled on some platforms.
* The clocksource code cannot be built on platforms that don't
use modern timekeeping but rely on gettimeoffset.

ACPI / thermal: fix workqueue destroy order
When the thermal module is to be removed, we should destroy the wq
acpi_thermal_pm_queue after the ACPI driver's remove callback is
executed as we will need to flush the workqueue there, or a NULL pointer
access will be hit.

MIPS: RC32434: fix broken PCI resource initialization
The parent field of the 'rc32434_res_pci_mem1' resource points to
the resource itself which is obviously wrong. Due to the broken
initialitazion, the PCI devices on the Mikrotik RB532 boards are
not working since commit
22283178 (MIPS: avoid possible resource
conflict in register_pci_controller).
Remove the field initialization to fix the issue.

ARM: 8064/1: fix v7-M signal return
According to the ARM ARM, the behaviour is UNPREDICTABLE if the PC read
from the exception return stack is not half word aligned. See the
pseudo code for ExceptionReturn() and PopStack().
The signal handler's address has the bit 0 set, and setup_return()
directly writes this to regs->ARM_pc. Current hardware happens to
discard this bit, but QEMU's emulation doesn't and this makes processes
crash. Mask out bit 0 before the exception return in order to get
predictable behaviour.

ARM: 8057/1: amba: Add Qualcomm vendor ID.
This patch adds Qualcomm amba vendor Id to the list. This ID is used in mmci driver. The ID selected in same lines like 0x41 is "A" for ARM, 0x51 is "Q" for Qualcomm.
As there are no physical register on Qcom SOC for amba vendor id, this is a fake ID assigned based on "Q" prefix from Qualcomm.

The arm EABI states that unwind opcode 10100nnn means pop register r4-4[4+nnn],aditionally there is a similar unwind opcode: 10101nnn which means the same thing plus popping r14. Those two cases are handled by the unwind_exec_pop_r4_to_rN function which checks whether the 4th bit is set and does r14 popping.
However, up until now it has been checking whether the 8th bit was set (mask & 0x80) instead of the 4th (mask & 0x8), a simple to make typo but this meant that we were always popping r14 even if we had the former opcode.
This patch changes the mask so that the 2 unwind opcodes are being handled correctly.

ARM: 8051/1: put_user: fix possible data corruption in put_user
According to arm procedure call standart r2 register is call-cloberred.
So after the result of x expression was put into r2 any following
function call in p may overwrite r2. To fix this, the result of p
expression must be saved to the temporary variable before the
assigment x expression to __r2.
