[SCSI] isci: remove unused 'isci_tmf->device' field
As the field was never set, isci_print_tmf() using 'isci_tmf->device'
sometimes causes a kernel crash if the dev_dbg() statement is enabled.
Remove the unused field both from isci_tmf struct definition and from
isci_print_tmf()

[SCSI] isci: performance-fix, shorten default "no outbound task" timeout
"No task timeout timer reduced from 20 to 2 This timer controls how
long the SCU hardware will hold open the TX side of the connection
before sending a DONE. The timer allows the hardware to attempt to
optimize the DONE/CLOSE behavior to allow for new COMMAND IU to be
posted. In practice closing the connection quicker is better."

[SCSI] isci: oem parameter format v1.3 (cable select)
v1.3 allows the attenuation of the attached cables to be specified to
the driver in terms of 'short', 'medium', and 'long' (see probe_roms.h).
These settings (per phy) are retrieved from the platform oem-parameters
(BIOS rom) or via a module parameter override.

[SCSI] isci: update afe (analog-front-end) recipe for C1
C1 silicon requires updates to the phy tuning recipe and also support
for user provided cable selects (per-phy) for short, medium, and long
cables. Default to 'short' awaiting support for selecting the cable via
oem parameters.

[SCSI] scsi_transport_fc: Clear Devloss Callback Done flag in fc_remote_port_rolechg
This patch fixes a bug where devloss is not called on fc_host teardown.
The issue is seen if the LLDD uses rport_rolechg to add the target role
to an rport.
When an rport goes away, the LLDD will call fc_remote_port_delete, which
will start the devloss timer. If the timer expires, the transport will
call the devloss callback and set the FC_RPORT_DEVLOSS_CALLBK_DONE flag.
However, the rport structure is not deleted, it is retained to store the
SCSI id mappings for the rport in case it comes back. In the scenario
where it does come back, and the driver calls fc_remote_port_add, but does
not indicate the "target" role for the rport - the create will clear the
structure, but forgets to clear FC_RPORT_DEVLOSS_CALLBK_DONE flag (which
is cleared if it's added with the target role). The secondary call, of
fc_remote_port_rolechg to add the target role also does not clear the flag.
Thus, the next time the rport goes away, the resulting devloss timer
expiration will not call the driver callback as the flag is still set.
This patch adds the FC_RPORT_DEVLOSS_CALLBK_DONE flags to the list of
those that are cleared upon reuse of the rport structure.

[SCSI] bfa: Implement LUN Masking feature using the SCSI Slave Callouts.
This patch re-implements LUN Masking feature using SCSI Slave Callouts. With
the new design in the slave_alloc entry point; for each new LUN discovered we
check with our internal LUN Masking config whether to expose or to mask this
particular LUN. We return -ENXIO (No such device or address) from slave_alloc
for the LUNs we don't want to be exposed. We also notify the SCSI mid-layer
to do a sequential LUN scan rather than REPORT_LUNS based scan if LUN masking
is enabled on our HBA port, since a -ENXIO from any LUN in REPORT_LUNS based
scan translates to a scan abort. This patch also handles the dynamic lun
masking config change from enable to disable or vice-versa by resetting
sdev_bflags of LUN 0 appropriately.

skb_linearize already has a check for skb_is_nonlinear,
there is no need to duplicate the check in fcoe.c. This

[SCSI] qla4xxx: Fixed BFS with sendtargets as boot index.
If ql4xdisablesysfsboot = 0 and sendtargets entry as boot index then
driver does export sendtarget entries in sysfs but iscsistart does not
do discovery. So in this case let driver do the discovery and
login to the targets.

[SCSI] qla4xxx: Limit the ACB Default Timeout value to 12s
A wrong default timeout value programmed in the adapter causes driver
to wait for that much time while waiting for target discoveries to complete.
This could add huge delays during the driver load time. To avoid this,
limit the default timeout value to 12 seconds if the default timeout value
set in adapter is less than 12 seconds and greater than 120 seconds.

[SCSI] libcxgbi: do not print a message when memory allocation fails
In alloc_pdu, libcxgbi tries to allocate a skb with GFP_ATOMIC, which
may potentially fail. When it happens, the current code prints a warning
message.
When the system is under IO stress, this failure may happen lots of
times and it usually scares users.
Instead of printing the warning message, the code now increases the
tx_dropped statistics for the ethernet interface wich is doing the iscsi
task.

drm/ttm: fix condition (and vs or)
The "if (!p && !p->dev)" condition isn't right because || was intended
instead of &&. But actually, "p" is the list cursor and so it's always
non-NULL and we can just remove that bit. We can remove the another
similar check as well.

dcache: use a dispose list in select_parent
select_parent currently abuses the dentry cache LRU to provide
cleanup features for child dentries that need to be freed. It moves
them to the tail of the LRU, then tells shrink_dcache_parent() to
calls __shrink_dcache_sb to unconditionally move them to a dispose
list (as DCACHE_REFERENCED is ignored). __shrink_dcache_sb() has to
relock the dentries to move them off the LRU onto the dispose list,
but otherwise does not touch the dentries that select_parent() moved
to the tail of the LRU. It then passses the dispose list to
shrink_dentry_list() which tries to free the dentries.
IOWs, the use of __shrink_dcache_sb() is superfluous - we can build
exactly the same list of dentries for disposal directly in
select_parent() and call shrink_dentry_list() instead of calling
__shrink_dcache_sb() to do that. This means that we avoid long holds
on the lru lock walking the LRU moving dentries to the dispose list
We also avoid the need to relock each dentry just to move it off the
LRU, reducing the numebr of times we lock each dentry to dispose of
them in shrink_dcache_parent() from 3 to 2 times.
Further, we remove one of the two callers of __shrink_dcache_sb().
This also means that __shrink_dcache_sb can be moved into back into
prune_dcache_sb() and we no longer have to handle referenced
dentries conditionally, simplifying the code.

tracing/mm: Move include of trace/events/kmem.h out of header into slab.c
Including trace/events/*.h TRACE_EVENT() macro headers in other headers
can cause strange side effects if another trace/event/*.h header
includes that header. Having trace/events/kmem.h inside slab_def.h
caused a compile error in sparc64 when changes were done to some header
files. Moving the kmem.h trace header out of slab.h and into slab.c
fixes the problem.
Note, both slub.c and slob.c already include the trace/events/kmem.h
file. Only slab.c had it missing.

igmp: Avoid zero delay when receiving odd mixture of IGMP queries
Commit
5b7c84066733c5dfb0e4016d939757b38de189e4 ('ipv4: correct IGMP
behavior on v3 query during v2-compatibility mode') added yet another
case for query parsing, which can result in max_delay = 0. Substitute
a value of 1, as in the usual v3 case.

net: introduce netif_addr_lock_nested() and call if when appropriate
dev_uc_sync() and dev_mc_sync() are acquiring netif_addr_lock for
destination device of synchronization. Since netif_addr_lock is
already held at the time for source device, this triggers lockdep
deadlock warning.
There's no way this deadlock can happen so use spin_lock_nested() to
silence the warning.

mtd: introduce mtd_can_have_bb helper
This patch introduces new 'mtd_can_have_bb()' helper function which checks
whether the flash can have bad eraseblocks. Then it changes all the
direct 'mtd->block_isbad' use cases with 'mtd_can_have_bb()'.

mtd: harmonize mtd_writev usage
This patch makes the 'mtd_writev()' function more usable and logical. We first
teach it to fall-back to the 'default_mtd_writev()' function if the MTD driver
does not define its own '->writev()' method. Then we make block2mtd and JFFS2
just 'mtd_writev()' instead of 'default_mtd_writev()' function. This means we
can now stop exporting 'default_mtd_writev()' and instead, export
'mtd_writev()'. This is much cleaner and more logical, as well as allows us to

mtd: do not use mtd->get_*_prot_info directly
Instead, call 'mtd_get_*_prot_info()' and check for '-EOPNOTSUPP'. While
on it, fix the return code from '-EOPNOTSUPP' to '-EINVAL' for the case
when the mode parameter is invalid.

mtd: do not use mtd->read_oob directly
Instead of checking whether 'mtd->read_oob' is defined, just call
'mtd_read_oob()' and handle the '-EOPNOTSUPP' error which will be returned
if the function is undefined.
Additionally, make 'mtd_write_oob()' return '-EOPNOTSUPP' if the function
is undefined.

romfs: do not use mtd->get_unmapped_area directly
Remove direct usage of mtd->get_unmapped_area. Instead, just call
'mtd_get_unmapped_area()' which will return -EOPNOTSUPP if the function
is not implemented, and then test for this code.
We also translate -EOPNOTSUPP to -ENOSYS because this return code is
probably part of the kernel ABI which we do not want to break.

mtd: introduce mtd_has_oob helper
We are working in the direction of making sure that MTD clients to not
use 'mtd->func' pointers directly. In some places we want to know if
OOB operations are supported by an MTD device. Introduce 'mtd_has_oob()'
helper for these purposes.

mtd: mtdcore: export symbols cleanup
The mtdcore.c file is a bit inconsistent - some EXPORT_SYMBOL_GPL declarations
follow the corresponding functions, some are placed at the end. This patch
harmonizes the file so that EXPORT_SYMBOL_GPL declarations follow the
corresponding function.
It also removes few extra newlines and trailing white-spaces.

mtd: clean-up the default_mtd_writev function
1. Teach 'mtd_write()' function to return '-EROFS' if the write method
is undefined, and remove the corresponding check from
'default_mtd_writev()'.
2. Do not test 'retlen' for NULL - it cannot be NULL.
3. Few minor coding stile clean-ups.
4. Add a kerneldoc comment
Additionally, minor fixes to the kerneldoc comments of the neighbor function.
