x86/fpu/math-emu: Remove !NO_UNDOC_CODE
We always want to support all FPU opcodes, including
undocumented ones. That define was fully justified ~20 years ago

x86/fpu: Check CPU-provided sizes against struct declarations
We now have C structures defined for each of the XSAVE state
components that we support. This patch adds checks during our
verification pass to ensure that the CPU-provided data
enumerated in CPUID leaves matches our C structures.
If not, we warn and dump all the XSAVE CPUID leaves.
Note: this *actually* found an inconsistency with the MPX
'bndcsr' state. The hardware pads it out differently from
our C structures. This patch caught it and warned.

x86/fpu: Check to ensure increasing-offset xstate offsets
The xstate CPUID leaves enumerate where each state component is
inside the XSAVE buffer, along with the size of the entire
buffer. Our new XSAVE sanity-checking code extrapolates an
expected _total_ buffer size by looking at the last component
that it encounters.
That method requires that the highest-numbered component also
be the one with the highest offset. This is a pretty safe
assumption, but let's add some code to ensure it stays true.
To make this check work correctly, we also need to ensure we
only consider the offsets from enabled features because the
offset register (ebx) will return 0 on unsupported features.
This also means that we will preserve the -1's that we
initialized xstate_offsets/sizes[] with. That will help
find bugs.

x86/fpu: Correct and check XSAVE xstate size calculations
Note: our xsaves support is currently broken and disabled. This
patch does not fix it, but it is an incremental improvement.
This might be useful to someone backporting the entire set of
XSAVES patches at some point, but it should not be backported
alone.

There are currently two xsave buffer formats: standard and
compacted. The standard format is waht 'XSAVE' and 'XSAVEOPT'
produce while 'XSAVES' and 'XSAVEC' produce a compacted-formet
buffer. (The kernel never uses XSAVEC)
But, the XSAVES buffer *ALSO* contains "system state components"
which are never saved by a plain XSAVE. So, XSAVES has two
things that might make its buffer differently-sized from an
XSAVE-produced one.
The current code assumes that an XSAVES buffer's size is simply
the sum of the sizes of the (user) states which are supported.
This seems to work in most cases, but it is not consistent with
what the SDM says, and it breaks if we 'align' a component in
the buffer. The calculation is also unnecessary work since the
CPU *tells* us the size of the buffer directly.
This patch just reads the size of the buffer right out of the
CPUID leaf instead of trying to derive it.
But, blindly trusting the CPU like this is dangerous. We add
a verification pass in do_extra_xstate_size_checks() to ensure
that the size we calculate matches with what we see from the
hardware. When it comes down to it, we trust but verify the
CPU.

x86/fpu: Add C structures for AVX-512 state components
AVX-512 has 3 separate state components:
1. opmask registers
2. zmm upper half of registers 0-15
3. new zmm registers (16-31)
This patch adds C structures for the three components along with
a few comments mostly lifted from the SDM to explain what they
do. This will allow us to check our structures against what the
hardware tells us about the sizes of the components.

x86/fpu: Rework YMM definition
We are about to rework all of the "extended state" definitions.
This makes the 'ymm' naming consistent with the AVX-512 types
we will introduce later.
We also add a convenience type: "reg_128_bit" so that we do
not have to spell out our arithmetic.

x86/fpu/mpx: Rework MPX 'xstate' types
MPX includes two separate "extended state components". There is
no real need to have an 'mpx_struct' because we never really
manage the states together.
We also separate out the actual data in 'mpx_bndcsr_state' from
the padding. We will shortly be checking the state sizes
against our structures and need them to match. For consistency,
we also ensure to prefix these types with 'mpx_'.
Lastly, we add some comments to mirror some of the descriptions
in the Intel documents (SDM) of the various state components.

x86/fpu: Add xfeature_enabled() helper instead of test_bit()
We currently use test_bit() in a few places to see if an
xfeature is enabled. It ends up being a bit ugly because
'xfeatures_mask' is a u64 and test_bit wants an 'unsigned long'
so it requires a cast. The *_bit() functions are also
techincally atomic, which we have no need for here.
So, remove the test_bit()s and replace with the new
xfeature_enabled() helper.
This also provides a central place to add a comment about the
future need to support 'system xstates'.

x86/fpu: Remove 'xfeature_nr'
xfeature_nr ended up being initialized too late for me to
use it in the "xsave size sanity check" patch which is
later in the series. I tried to move around its initialization
but realized that it was just as easy to get rid of it.
We only have 9 XFEATURES. Instead of dynamically calculating
and storing the last feature, just use the compile-time max:
XFEATURES_NR_MAX. Note that even with 'xfeatures_nr' we can
had "holes" in the xfeatures_mask that we had to deal with.
We also change a 'leaf' variable to be a plain 'i'. Although
it is used to grab a cpuid leaf in this one loop, all of the
other loops just use an 'i' and I find it much more obvious
to keep the naming consistent across all the similar loops.

x86/fpu: Rework XSTATE_* macros to remove magic '2'
The 'xstate.c' code has a bunch of references to '2'. This
is because we have a lot more work to do for the "extended"
xstates than the "legacy" ones and state component 2 is the
first "extended" state.
This patch replaces all of the instances of '2' with
FIRST_EXTENDED_XFEATURE, which clearly explains what is
going on.

x86/fpu: Rename XSAVE macros
There are two concepts that have some confusing naming:
1. Extended State Component numbers (currently called
XFEATURE_BIT_*)
2. Extended State Component masks (currently called XSTATE_*)
The numbers are (currently) from 0-9. State component 3 is the
bounds registers for MPX, for instance.
But when we want to enable "state component 3", we go set a bit
in XCR0. The bit we set is 1<<3. We can check to see if a
state component feature is enabled by looking at its bit.
The current 'xfeature_bit's are at best xfeature bit _numbers_.
Calling them bits is at best inconsistent with ending the enum
list with 'XFEATURES_NR_MAX'.
This patch renames the enum to be 'xfeature'. These also
happen to be what the Intel documentation calls a "state
component".
We also want to differentiate these from the "XSTATE_*" macros.
The "XSTATE_*" macros are a mask, and we rename them to match.
These macros are reasonably widely used so this patch is a
wee bit big, but this really is just a rename.
The only non-mechanical part of this is the
s/XSTATE_EXTEND_MASK/XFEATURE_MASK_EXTEND/
We need a better name for it, but that's another patch.

x86/fpu: Remove XSTATE_RESERVE
The original purpose of XSTATE_RESERVE was to carve out space
to store all of the possible extended state components that
get saved with the XSAVE instruction(s).
However, we are now almost entirely dynamically allocating
the buffers we use for XSAVE by placing them at the end of
the task_struct and them sizing them at boot. The one
exception for that is the init_task.
The maximum extended state component size that we have today
is on systems with space for AVX-512 and Memory Protection
Keys: 2696 bytes. We have reserved a PAGE_SIZE buffer in
the init_task via fpregs_state->__padding.
This check ensures that even if the component sizes or
layout were changed (which we do not expect), that we will
still not overflow the init_task's buffer.
In the case that we detect we might overflow the buffer,
we completely disable XSAVE support in the kernel and try
to boot as if we had 'legacy x87 FPU' support in place.
This is a crippled state without any of the XSAVE-enabled
features (MPX, AVX, etc...). But, it at least let us
boot safely.

x86/fpu: Move XSAVE-disabling code to a helper
When we want to _completely_ disable XSAVE support as far as
the kernel is concerned, we have a big set of feature flags
to clear. We currently only do this in cases where the user
asks for it to be disabled, but we are about to expand the
places where we do it to handle errors too.
Move the code in to xstate.c, and put it in the xstate.h
header. We will use it in the next patch too.

x86/fpu: Print xfeature buffer size in decimal
This is utterly a personal taste thing, but I find it way easier
to read structure sizes in decimal than in hex.

blk: rq_data_dir() should not return a boolean
rq_data_dir() returns either READ or WRITE (0 == READ, 1 == WRITE), not
a boolean value.
Now, admittedly the "!= 0" doesn't really change the value (0 stays as
zero, 1 stays as one), but it's not only redundant, it confuses gcc, and
causes gcc to warn about the construct
switch (rq_data_dir(req)) {
case READ:
...
case WRITE:
...
that we have in a few drivers.
Now, the gcc warning is silly and stupid (it seems to warn not about the
switch value having a different type from the case statements, but about
_any_ boolean switch value), but in this case the code itself is silly
and stupid too, so let's just change it, and get rid of warnings like
this:

The odd '!= 0' came in when "cmd_flags" got turned into a "u64" in
commit
5953316dbf90 ("block: make rq->cmd_flags be 64-bit") and is
presumably because the old code (that just did a logical 'and' with 1)
would then end up making the type of rq_data_dir() be u64 too.
But if we want to retain the old regular integer type, let's just cast
the result to 'int' rather than use that rather odd '!= 0'.

We had to revert the pluggin in writeback_sb_inodes() because the
wb->list_lock is held, but we could easily plug at a higher level before
taking that lock, and unplug after releasing it. This does that.
Chris will run performance numbers, just to verify that this approach is
comparable to the alternative (we could just drop and re-take the lock
around the blk_finish_plug() rather than these two commits.
I'd have preferred waiting for actual performance numbers before picking
one approach over the other, but I don't want to release rc1 with the
known "sleeping function called from invalid context" issue, so I'll
pick this cleanup version for now. But if the numbers show that we
really want to plug just at the writeback_sb_inodes() level, and we
should just play ugly games with the spinlock, we'll switch to that.

thermal: fix intel PCH thermal driver mismerge
I didn't notice this when merging the thermal code from Zhang, but his
merge (commit
5a924a07f882: "Merge branches 'thermal-core' and
'thermal-intel' of .git into next") of the thermal-core and
thermal-intel branches was wrong.
In thermal-core, commit
17e8351a7739 ("thermal: consistently use int for
temperatures") converted the thermal layer to use "int" for
temperatures.
But in parallel, in the thermal-intel branch commit
d0a12625d2ff
("thermal: Add Intel PCH thermal driver") added support for the intel
PCH thermal sensor using the old interfaces that used "unsigned long"
pointers.
This resulted in warnings like this:

mm/early_ioremap: add explicit #include of asm/early_ioremap.h
Commit
6b0f68e32ea8 ("mm: add utility for early copy from unmapped ram")
introduces a function copy_from_early_mem() into mm/early_ioremap.c
which itself calls early_memremap()/early_memunmap(). However, since
early_memunmap() has not been declared yet at this point in the .c file,
nor by any explicitly included header files, we are depending on a
transitive include of asm/early_ioremap.h to declare it, which is
fragile.
So instead, include this header explicitly.

fs/seq_file: convert int seq_vprint/seq_printf/etc... returns to void
The seq_<foo> function return values were frequently misused.

All uses of these return values have been removed, so convert the
return types to void.

sys_membarrier(): system-wide memory barrier (generic, x86)
Here is an implementation of a new system call, sys_membarrier(), which
executes a memory barrier on all threads running on the system. It is
implemented by calling synchronize_sched(). It can be used to
distribute the cost of user-space memory barriers asymmetrically by
transforming pairs of memory barriers into pairs consisting of
sys_membarrier() and a compiler barrier. For synchronization primitives
that distinguish between read-side and write-side (e.g. userspace RCU
[1], rwlocks), the read-side can be accelerated significantly by moving
the bulk of the memory barrier overhead to the write-side.
The existing applications of which I am aware that would be improved by
this system call are as follows:

Those projects use RCU in userspace to increase read-side speed and
scalability compared to locking. Especially in the case of RCU used by
libraries, sys_membarrier can speed up the read-side by moving the bulk of
the memory barrier cost to synchronize_rcu().
* Direct users of sys_membarrier

Microsoft core dotnet GC developers are planning to use the mprotect()
side-effect of issuing memory barriers through IPIs as a way to implement
Windows FlushProcessWriteBuffers() on Linux. They are referring to
sys_membarrier in their github thread, specifically stating that
sys_membarrier() is what they are looking for.
To explain the benefit of this scheme, let's introduce two example threads:
Thread A (non-frequent, e.g. executing liburcu synchronize_rcu())
Thread B (frequent, e.g. executing liburcu
rcu_read_lock()/rcu_read_unlock())
In a scheme where all smp_mb() in thread A are ordering memory accesses
with respect to smp_mb() present in Thread B, we can change each
smp_mb() within Thread A into calls to sys_membarrier() and each
smp_mb() within Thread B into compiler barriers "barrier()".
Before the change, we had, for each smp_mb() pairs:

As we can see, there are two possible scenarios: either Thread B memory
accesses do not happen concurrently with Thread A accesses (1), or they
do (2).
1) Non-concurrent Thread A vs Thread B accesses:

In this case, thread B accesses will be weakly ordered. This is OK,
because at that point, thread A is not particularly interested in
ordering them with respect to its own accesses.

In this case, thread B accesses, which are ensured to be in program
order thanks to the compiler barrier, will be "upgraded" to full
smp_mb() by synchronize_sched().

On Intel Xeon E5405 (8 cores)
(one thread is calling sys_membarrier, the other 7 threads are busy
looping)
1000 non-expedited sys_membarrier calls in 33s =3D 33 milliseconds/call.
* User-space user of this system call: Userspace RCU library
Both the signal-based and the sys_membarrier userspace RCU schemes
permit us to remove the memory barrier from the userspace RCU
rcu_read_lock() and rcu_read_unlock() primitives, thus significantly
accelerating them. These memory barriers are replaced by compiler
barriers on the read-side, and all matching memory barriers on the
write-side are turned into an invocation of a memory barrier on all
active threads in the process. By letting the kernel perform this
synchronization rather than dumbly sending a signal to every process
threads (as we currently do), we diminish the number of unnecessary wake
ups and only issue the memory barriers on active threads. Non-running
threads do not need to execute such barrier anyway, because these are
implied by the scheduler context switches.
Results in liburcu:
Operations in 10s, 6 readers, 2 writers:

sys_membarrier:
9952759104 reads, 425 writes
sys_membarrier (dyn. check):

The dynamic sys_membarrier availability check adds some overhead to
the read-side compared to the signal-based scheme, but besides that,
sys_membarrier slightly outperforms the signal-based scheme. However,
this non-expedited sys_membarrier implementation has a much slower grace
period than signal and memory barrier schemes.
Besides diminishing the number of wake-ups, one major advantage of the
membarrier system call over the signal-based scheme is that it does not
need to reserve a signal. This plays much more nicely with libraries,
and with processes injected into for tracing purposes, for which we
cannot expect that signals will be unused by the application.
An expedited version of this system call can be added later on to speed
up the grace period. Its implementation will likely depend on reading
the cpu_curr()->mm without holding each CPU's rq lock.
This patch adds the system call to x86 and to asm-generic.

Query the set of supported commands. It returns a bitmask of

Execute a memory barrier on all threads running on the system.
Upon return from system call, the caller thread is ensured that
all running threads have passed through a state where all memory
accesses to user-space addresses match program order between
entry to and return from the system call (non-running threads
are de facto in such a state). This covers threads from all pro=E2=80=90
cesses running on the system. This command returns 0.
The flags argument needs to be 0. For future extensions.
All memory accesses performed in program order from each targeted
thread is guaranteed to be ordered with respect to sys_membarrier(). If
we use the semantic "barrier()" to represent a compiler barrier forcing
memory accesses to be performed in program order across the barrier,
and smp_mb() to represent explicit memory barriers forcing full memory
ordering across the barrier, we have the following ordering table for
each pair of barrier(), sys_membarrier() and smp_mb():

RETURN VALUE
On success, these system calls return zero. On error, -1 is returned,
and errno is set appropriately. For a given command, with flags
argument set to 0, this system call is guaranteed to always return the
same value until reboot.

ENOSYS System call is not implemented.
EINVAL Invalid arguments.

Revert "writeback: plug writeback at a high level"
This reverts commit
d353d7587d02116b9732d5c06615aed75a4d3a47.
Doing the block layer plug/unplug inside writeback_sb_inodes() is
broken, because that function is actually called with a spinlock held:
wb->list_lock, as pointed out by Chris Mason.
Chris suggested just dropping and re-taking the spinlock around the
blk_finish_plug() call (the plgging itself can happen under the
spinlock), and that would technically work, but is just disgusting.
We do something fairly similar - but not quite as disgusting because we
at least have a better reason for it - in writeback_single_inode(), so
it's not like the caller can depend on the lock being held over the
call, but in this case there just isn't any good reason for that
"release and re-take the lock" pattern.
[ In general, we should really strive to avoid the "release and retake"
pattern for locks, because in the general case it can easily cause
subtle bugs when the caller caches any state around the call that
might be invalidated by dropping the lock even just temporarily. ]
But in this case, the plugging should be easy to just move up to the
callers before the spinlock is taken, which should even improve the
effectiveness of the plug. So there is really no good reason to play
games with locking here.
I'll send off a test-patch so that Dave Chinner can verify that that
plug movement works. In the meantime this just reverts the problematic
commit and adds a comment to the function so that we hopefully don't
make this mistake again.

Calling transport_generic_request_failure() from here causes list
corruption. We should be using target_complete_cmd() instead.
Which we do in all other cases, so the UNKNOWN_OP case can become just
another member of the big else/if chain in tcmu_handle_completion().

target: Attach EXTENDED_COPY local I/O descriptors to xcopy_pt_sess
This patch is a >= v4.1 regression bug-fix where control CDB
emulation logic in commit
38b57f82 now expects a se_cmd->se_sess
pointer to exist when determining T10-PI support is to be exposed
for initiator host ports.
To address this bug, go ahead and add locally generated se_cmd
descriptors for copy-offload block-copy to it's own stand-alone
se_session nexus, while the parent EXTENDED_COPY se_cmd descriptor
remains associated with it's originating se_cmd->se_sess nexus.
Note a valid se_cmd->se_sess is also required for future support
of WRITE_INSERT and READ_STRIP software emulation when submitting
backend I/O to se_device that exposes T10-PI suport.

target/qla2xxx: Honor max_data_sg_nents I/O transfer limit
This patch adds an optional fabric driver provided SGL limit
that target-core will honor as it's own internal I/O maximum
transfer length limit, as exposed by EVPD=0xb0 block limits
parameters.
This is required for handling cases when host I/O transfer
length exceeds the requested EVPD block limits maximum
transfer length. The initial user of this logic is qla2xxx,
so that we can avoid having to reject I/Os from some legacy
FC hosts where EVPD=0xb0 parameters are not honored.
When se_cmd payload length exceeds the provided limit in
target_check_max_data_sg_nents() code, se_cmd->data_length +
se_cmd->prot_length are reset with se_cmd->residual_count
plus underflow bit for outgoing TFO response callbacks.
It also checks for existing CDB level underflow + overflow
and recalculates final residual_count as necessary.
Note this patch currently assumes 1:1 mapping of PAGE_SIZE
per struct scatterlist entry.

"A bit later in the cycle, but this has been in the block tree for a a
while. This is basically four patchsets from Tejun, that improve our
buffered cgroup writeback. It was dependent on the other cgroup
changes, but they went in earlier in this cycle.
Series 1 is set of 5 patches that has cgroup writeback updates:
- bdi_writeback iteration fix which could lead to some wb's being
skipped or repeated during e.g. sync under memory pressure.
- Simplification of wb work wait mechanism.
- Writeback tracepoints updated to report cgroup.
Series 2 is is a set of updates for the CFQ cgroup writeback handling:
cfq has always charged all async IOs to the root cgroup. It didn't
have much choice as writeback didn't know about cgroups and there
was no way to tell who to blame for a given writeback IO.
writeback finally grew support for cgroups and now tags each
writeback IO with the appropriate cgroup to charge it against.
This patchset updates cfq so that it follows the blkcg each bio is
tagged with. Async cfq_queues are now shared across cfq_group,
which is per-cgroup, instead of per-request_queue cfq_data. This
makes all IOs follow the weight based IO resource distribution
implemented by cfq.
- Switched from GFP_ATOMIC to GFP_NOWAIT as suggested by Jeff.
- Other misc review points addressed, acks added and rebased.
Series 3 is the blkcg policy cleanup patches:
This patchset contains assorted cleanups for blkcg_policy methods
and blk[c]g_policy_data handling.
- alloc/free added for blkg_policy_data. exit dropped.
- alloc/free added for blkcg_policy_data.
- blk-throttle's async percpu allocation is replaced with direct
allocation.
- all methods now take blk[c]g_policy_data instead of blkcg_gq or
blkcg.
And finally, series 4 is a set of patches cleaning up the blkcg stats
handling:
blkcg's stats have always been somwhat of a mess. This patchset
tries to improve the situation a bit.
- The following patches added to consolidate blkcg entry point and
blkg creation. This is in itself is an improvement and helps
colllecting common stats on bio issue.
- per-blkg stats now accounted on bio issue rather than request
completion so that bio based and request based drivers can behave
the same way. The issue was spotted by Vivek.
- cfq-iosched implements custom recursive stats and blk-throttle
implements custom per-cpu stats. This patchset make blkcg core
support both by default.
- cfq-iosched and blk-throttle keep track of the same stats
multiple times. Unify them"

drm/qxl: validate monitors config modes
Due to some recent changes in
drm_helper_probe_single_connector_modes_merge_bits(), old custom modes
were not being pruned properly. In current kernels,
drm_mode_validate_basic() is called to sanity-check each mode in the
list. If the sanity-check passes, the mode's status gets set to to
MODE_OK. In older kernels this check was not done, so old custom modes
would still have a status of MODE_UNVERIFIED at this point, and would
therefore be pruned later in the function.
As a result of this new behavior, the list of modes for a device always
includes every custom mode ever configured for the device, with the
largest one listed first. Since desktop environments usually choose the
first preferred mode when a hotplug event is emitted, this had the
result of making it very difficult for the user to reduce the size of
the display.
The qxl driver did implement the mode_valid connector function, but it
was empty. In order to restore the old behavior where old custom modes
are pruned, we implement a proper mode_valid function for the qxl
driver. This function now checks each mode against the last configured
custom mode and the list of standard modes. If the mode doesn't match
any of these, its status is set to MODE_BAD so that it will be pruned as
expected.

dma-mapping: consolidate dma_set_mask
Almost everyone implements dma_set_mask the same way, although some time
that's hidden in ->set_dma_mask methods.
This patch consolidates those into a common implementation that either
calls ->set_dma_mask if present or otherwise uses the default
implementation. Some architectures used to only call ->set_dma_mask
after the initial checks, and those instance have been fixed to do the
full work. h8300 implemented dma_set_mask bogusly as a no-ops and has
been fixed.
Unfortunately some architectures overload unrelated semantics like changing
the dma_ops into it so we still need to allow for an architecture override
for now.

dma-mapping: consolidate dma_supported
Most architectures just call into ->dma_supported, but some also return 1
if the method is not present, or 0 if no dma ops are present (although
that should never happeb). Consolidate this more broad version into
common code.
Also fix h8300 which inorrectly always returned 0, which would have been
a problem if it's dma_set_mask implementation wasn't a similarly buggy
noop.
As a few architectures have much more elaborate implementations, we
still allow for arch overrides.

dma-mapping: cosolidate dma_mapping_error
Currently there are three valid implementations of dma_mapping_error:
(1) call ->mapping_error
(2) check for a hardcoded error code
(3) always return 0
This patch provides a common implementation that calls ->mapping_error
if present, then checks for DMA_ERROR_CODE if defined or otherwise
returns 0.

dma-mapping: consolidate dma_{alloc,free}_noncoherent
Most architectures do not support non-coherent allocations and either
define dma_{alloc,free}_noncoherent to their coherent versions or stub
them out.
Openrisc uses dma_{alloc,free}_attrs to implement them, and only Mips
implements them directly.
This patch moves the Openrisc version to common code, and handles the
DMA_ATTR_NON_CONSISTENT case in the mips dma_map_ops instance.
Note that actual non-coherent allocations require a dma_cache_sync
implementation, so if non-coherent allocations didn't work on
an architecture before this patch they still won't work after it.

dma-mapping: consolidate dma_{alloc,free}_{attrs,coherent}
Since 2009 we have a nice asm-generic header implementing lots of DMA API
functions for architectures using struct dma_map_ops, but unfortunately
it's still missing a lot of APIs that all architectures still have to
duplicate.
This series consolidates the remaining functions, although we still need
arch opt outs for two of them as a few architectures have very
non-standard implementations.
This patch (of 5):
The coherent DMA allocator works the same over all architectures supporting
dma_map operations.
This patch consolidates them and converges the minor differences:
- the debug_dma helpers are now called from all architectures, including
those that were previously missing them
- dma_alloc_from_coherent and dma_release_from_coherent are now always
called from the generic alloc/free routines instead of the ops
dma-mapping-common.h always includes dma-coherent.h to get the defintions
for them, or the stubs if the architecture doesn't support this feature
- checks for ->alloc / ->free presence are removed. There is only one
magic instead of dma_map_ops without them (mic_dma_ops) and that one
is x86 only anyway.
Besides that only x86 needs special treatment to replace a default devices
if none is passed and tweak the gfp_flags. An optional arch hook is provided
for that.

mm: make sure all file VMAs have ->vm_ops set
We rely on vma->vm_ops == NULL to detect anonymous VMA: see
vma_is_anonymous(), but some drivers doesn't set ->vm_ops.
As a result we can end up with anonymous page in private file mapping.
That should not lead to serious misbehaviour, but nevertheless is wrong.
Let's fix by setting up dummy ->vm_ops for file mmapping if f_op->mmap()
didn't set its own.
The patch also adds sanity check into __vma_link_rb(). It will help
catch broken VMAs which inserted directly into mm_struct via
insert_vm_struct().

mm, mpx: add "vm_flags_t vm_flags" arg to do_mmap_pgoff()
Add the additional "vm_flags_t vm_flags" argument to do_mmap_pgoff(),
rename it to do_mmap(), and re-introduce do_mmap_pgoff() as a simple
wrapper on top of do_mmap(). Perhaps we should update the callers of
do_mmap_pgoff() and kill it later.
This way mpx_mmap() can simply call do_mmap(vm_flags => VM_MPX) and do not
play with vm internals.
After this change mmap_region() has a single user outside of mmap.c,
arch/tile/mm/elf.c:arch_setup_additional_pages(). It would be nice to
change arch/tile/ and unexport mmap_region().

ipc: convert invalid scenarios to use WARN_ON
Considering Linus' past rants about the (ab)use of BUG in the kernel, I
took a look at how we deal with such calls in ipc. Given that any errors
or corruption in ipc code are most likely contained within the set of
processes participating in the broken mechanisms, there aren't really many
strong fatal system failure scenarios that would require a BUG call.
Also, if something is seriously wrong, ipc might not be the place for such
a BUG either.
1. For example, recently, a customer hit one of these BUG_ONs in shm
after failing shm_lock(). A busted ID imho does not merit a BUG_ON,
and WARN would have been better.
2. MSG_COPY functionality of posix msgrcv(2) for checkpoint/restore.
I don't see how we can hit this anyway -- at least it should be IS_ERR.
The 'copy' arg from do_msgrcv is always set by calling prepare_copy()
first and foremost. We could also probably drop this check altogether.
Either way, it does not merit a BUG_ON.
3. No ->fault() callback for the fs getting the corresponding page --
seems selfish to make the system unusable.

the new buffer is at 0x46fe000000ULL, decompressor_gzip is using
0xffffffb901ffffff as out_len. gunzip in lib/zlib_inflate/inflate.c cap
that len to 0x01ffffff and decompress fails later.
We could hit this problem with crashkernel booting that uses kexec loading
kernel above 4GiB.
We have decompress_* support:
1. inbuf[]/outbuf[] for kernel preboot.
2. inbuf[]/flush() for initramfs
3. fill()/flush() for initrd.
This bug only affect kernel preboot path that use outbuf[].
Add __decompress and take real out_buf_len for gunzip instead of guessing
wrong buf size.

kexec: export KERNEL_IMAGE_SIZE to vmcoreinfo
In x86_64, since v2.6.26 the KERNEL_IMAGE_SIZE is changed to 512M, and
accordingly the MODULES_VADDR is changed to 0xffffffffa0000000. However,
in v3.12 Kees Cook introduced kaslr to randomise the location of kernel.
And the kernel text mapping addr space is enlarged from 512M to 1G. That
means now KERNEL_IMAGE_SIZE is variable, its value is 512M when kaslr
support is not compiled in and 1G when kaslr support is compiled in.
Accordingly the MODULES_VADDR is changed too to be:
#define MODULES_VADDR (__START_KERNEL_map + KERNEL_IMAGE_SIZE)
So when kaslr is compiled in and enabled, the kernel text mapping addr
space and modules vaddr space need be adjusted. Otherwise makedumpfile
will collapse since the addr for some symbols is not correct.
Hence KERNEL_IMAGE_SIZE need be exported to vmcoreinfo and got in
makedumpfile to help calculate MODULES_VADDR.

kexec: align crash_notes allocation to make it be inside one physical page
People reported that crash_notes in /proc/vmcore were corrupted and this
cause crash kdump failure. With code debugging and log we got the root
cause. This is because percpu variable crash_notes are allocated in 2
vmalloc pages. Currently percpu is based on vmalloc by default. Vmalloc
can't guarantee 2 continuous vmalloc pages are also on 2 continuous
physical pages. So when 1st kernel exports the starting address and size
of crash_notes through sysfs like below:
/sys/devices/system/cpu/cpux/crash_notes
/sys/devices/system/cpu/cpux/crash_notes_size
kdump kernel use them to get the content of crash_notes. However the 2nd
part may not be in the next neighbouring physical page as we expected if
crash_notes are allocated accross 2 vmalloc pages. That's why
nhdr_ptr->n_namesz or nhdr_ptr->n_descsz could be very huge in
update_note_header_size_elf64() and cause note header merging failure or
some warnings.
In this patch change to call __alloc_percpu() to passed in the align value
by rounding crash_notes_size up to the nearest power of two. This makes
sure the crash_notes is allocated inside one physical page since
sizeof(note_buf_t) in all ARCHS is smaller than PAGE_SIZE. Meanwhile add
a BUILD_BUG_ON to break compile if size is bigger than PAGE_SIZE since
crash_notes definitely will be in 2 pages. That need be avoided, and need
be reported if it's unavoidable.

kexec: split kexec_load syscall from kexec core code
There are two kexec load syscalls, kexec_load another and kexec_file_load.
kexec_file_load has been splited as kernel/kexec_file.c. In this patch I
split kexec_load syscall code to kernel/kexec.c.
And add a new kconfig option KEXEC_CORE, so we can disable kexec_load and
use kexec_file_load only, or vice verse.
The original requirement is from Ted Ts'o, he want kexec kernel signature
being checked with CONFIG_KEXEC_VERIFY_SIG enabled. But kexec-tools use
kexec_load syscall can bypass the checking.
Vivek Goyal proposed to create a common kconfig option so user can compile
in only one syscall for loading kexec kernel. KEXEC/KEXEC_FILE selects
KEXEC_CORE so that old config files still work.
Because there's general code need CONFIG_KEXEC_CORE, so I updated all the
architecture Kconfig with a new option KEXEC_CORE, and let KEXEC selects

kexec: split kexec_file syscall code to kexec_file.c
Split kexec_file syscall related code to another file kernel/kexec_file.c
so that the #ifdef CONFIG_KEXEC_FILE in kexec.c can be dropped.
Sharing variables and functions are moved to kernel/kexec_internal.h per
suggestion from Vivek and Petr.

parisc: use seq_hex_dump() to dump buffers
Instead of custom approach let's use recently introduced seq_hex_dump()
helper.
In one case it changes the output from
1111111122222222333333334444444455555555666666667777777788888888
to
11111111 22222222 33333333 44444444 55555555 66666666 77777777 88888888
though it seems it prints same data (by meaning) in both cases. I decide
to choose to use the space divided one.
