KVM: x86: fix KVM_SET_XCRS loop
The loop was always using 0 as the index. This means that
any rubbish after the first element of the array went undetected.
It seems reasonable to assume that no KVM userspace did that.

KVM: x86: fix KVM_SET_XCRS for CPUs that do not support XSAVE
The KVM_SET_XCRS ioctl must accept anything that KVM_GET_XCRS
could return. XCR0's bit 0 is always 1 in real processors with
XSAVE, and KVM_GET_XCRS will always leave bit 0 set even if the
emulated processor does not have XSAVE. So, KVM_SET_XCRS must
ignore that bit when checking for attempts to enable unsupported
save states.

kvm: Create non-coherent DMA registeration
We currently use some ad-hoc arch variables tied to legacy KVM device
assignment to manage emulation of instructions that depend on whether
non-coherent DMA is present. Create an interface for this, adapting
legacy KVM device assignment and adding VFIO via the KVM-VFIO device.
For now we assume that non-coherent DMA is possible any time we have a
VFIO group. Eventually an interface can be developed as part of the
VFIO external user interface to query the coherency of a group.

kvm: Add VFIO device
So far we've succeeded at making KVM and VFIO mostly unaware of each
other, but areas are cropping up where a connection beyond eventfds
and irqfds needs to be made. This patch introduces a KVM-VFIO device
that is meant to be a gateway for such interaction. The user creates
the device and can add and remove VFIO groups to it via file
descriptors. When a group is added, KVM verifies the group is valid
and gets a reference to it via the VFIO external user interface.

kvm: Emulate MOVBE
This basically came from the need to be able to boot 32-bit Atom SMP
guests on an AMD host, i.e. a host which doesn't support MOVBE. As a
matter of fact, qemu has since recently received MOVBE support but we
cannot share that with kvm emulation and thus we have to do this in the
host. We're waay faster in kvm anyway. :-)
So, we piggyback on the #UD path and emulate the MOVBE functionality.
With it, an 8-core SMP guest boots in under 6 seconds.
Also, requesting MOVBE emulation needs to happen explicitly to work,
i.e. qemu -cpu n270,+movbe...
Just FYI, a fairly straight-forward boot of a MOVBE-enabled 3.9-rc6+
kernel in kvm executes MOVBE ~60K times.

kvm: Add KVM_GET_EMULATED_CPUID
Add a kvm ioctl which states which system functionality kvm emulates.
The format used is that of CPUID and we return the corresponding CPUID
bits set for which we do emulate functionality.
Make sure ->padding is being passed on clean from userspace so that we
can use it for something in the future, after the ioctl gets cast in
stone.
s/kvm_dev_ioctl_get_supported_cpuid/kvm_dev_ioctl_get_cpuid/ while at
it.

KVM: Mapping IOMMU pages after updating memslot
In kvm_iommu_map_pages(), we need to know the page size via call
kvm_host_page_size(). And it will check whether the target slot
is valid before return the right page size.
Currently, we will map the iommu pages when creating a new slot.
But we call kvm_iommu_map_pages() during preparing the new slot.
At that time, the new slot is not visible by domain(still in preparing).
So we cannot get the right page size from kvm_host_page_size() and
this will break the IOMMU super page logic.
The solution is to map the iommu pages after we insert the new slot
into domain.

nVMX: Fix pick-up of uninjected NMIs
__vmx_complete_interrupts stored uninjected NMIs in arch.nmi_injected,
not arch.nmi_pending. So we actually need to check the former field in
vmcs12_save_pending_event. This fixes the eventinj unit test when run
in nested KVM.

KVM: ARM: Transparent huge page (THP) support
Support transparent huge pages in KVM/ARM and KVM/ARM64. The
transparent_hugepage_adjust is not very pretty, but this is also how
it's solved on x86 and seems to be simply an artifact on how THPs
behave. This should eventually be shared across architectures if
possible, but that can always be changed down the road.

KVM: ARM: Support hugetlbfs backed huge pages
Support huge pages in KVM/ARM and KVM/ARM64. The pud_huge checking on
the unmap path may feel a bit silly as the pud_huge check is always
defined to false, but the compiler should be smart about this.
Note: This deals only with VMAs marked as huge which are allocated by
users through hugetlbfs only. Transparent huge pages can only be
detected by looking at the underlying pages (or the page tables
themselves) and this patch so far simply maps these on a page-by-page
level in the Stage-2 page tables.

ARM: KVM: Yield CPU when vcpu executes a WFE
On an (even slightly) oversubscribed system, spinlocks are quickly
becoming a bottleneck, as some vcpus are spinning, waiting for a
lock to be released, while the vcpu holding the lock may not be
running at all.
This creates contention, and the observed slowdown is 40x for
hackbench. No, this isn't a typo.
The solution is to trap blocking WFEs and tell KVM that we're
now spinning. This ensures that other vpus will get a scheduling
boost, allowing the lock to be released more quickly. Also, using
CONFIG_HAVE_KVM_CPU_RELAX_INTERCEPT slightly improves the performance
when the VM is severely overcommited.
Quick test to estimate the performance: hackbench 1 process 1000
2xA15 host (baseline): 1.843s
2xA15 guest w/o patch: 2.083s
4xA15 guest w/o patch: 80.212s
8xA15 guest w/o patch: Could not be bothered to find out
2xA15 guest w/ patch: 2.102s
4xA15 guest w/ patch: 3.205s
8xA15 guest w/ patch: 6.887s
So we go from a 40x degradation to 1.5x in the 2x overcommit case,
which is vaguely more acceptable.

We use jump label to enable pv-spinlock. With the changes in (
442e0973e927
Merge branch 'x86/jumplabel'), the jump label behaviour has changed
that would result in eventual hang of the VM since we would end up in a
situation where slow path locks would halt the vcpus but we will not be
able to wakeup the vcpu by lock releaser using unlock kick.
Similar problem in Xen and more detailed description is available in
a945928ea270 (xen: Do not enable spinlocks before jump_label_init()
has executed)
This patch splits kvm_spinlock_init to separate jump label changes with
pvops patching and also make jump label enabling after jump_label_init().

KVM: Drop FOLL_GET in GUP when doing async page fault
Page pinning is not mandatory in kvm async page fault processing since
after async page fault event is delivered to a guest it accesses page once
again and does its own GUP. Drop the FOLL_GET flag in GUP in async_pf
code, and do some simplifying in check/clear processing.

Tim Bird expressed concern that this will have a bad effect on boot
time, and while simple tests have shown it to be okay with simple tree,
a device tree blob can potentially be quite large and
add_device_randomness() is not a fast function. Rather than do this for
all platforms unconditionally, I'm reverting this patch and would like
to see it revisited. Instead of feeding the entire tree into the random
pool, it would probably be appropriate to hash the tree and feed the
hash result into the pool. There really isn't a lot of randomness in a
device tree anyway. In the majority of cases only a handful of
properties are going to be different between machines with the same
baseboard.

of: fix unnecessary warning on missing /cpus node
Not all DT platforms have all the cpus collected under a /cpus node.
That just happens to be a details of FDT, ePAPR and PowerPC platforms.
Sparc does something different, but unfortunately the current code
complains with a warning if /cpus isn't there. This became a problem
with commit
f86e4718, "driver/core cpu: initialize of_node in cpu's
device structure", which caused the function to get called for all
architectures.
This commit is a temporary fix to fail silently if the cpus node isn't
present. A proper fix will come later to allow arch code to provide a
custom mechanism for decoding the CPU hwid if the 'reg' property isn't
appropriate.

KVM: Move gfn_to_index to x86 specific code
The gfn_to_index function relies on huge page defines which either may
not make sense on systems that don't support huge pages or are defined
in an unconvenient way for other architectures. Since this is
x86-specific, move the function to arch/x86/include/asm/kvm_host.h.

In ftrace_syscall_enter(),
syscall_get_arguments(..., 0, n, ...)
if (i == 0) { <handle ORIG_r0> ...; n--;}
memcpy(..., n * sizeof(args[0]));
If 'number of arguments(n)' is zero and 'argument index(i)' is also zero in
syscall_get_arguments(), none of arguments should be copied by memcpy().
Otherwise 'n--' can be a big positive number and unexpected amount of data
will be copied. Tracing system calls which take no argument, say sync(void),
may hit this case and eventually make the system corrupted.
This patch fixes the issue both in syscall_get_arguments() and
syscall_set_arguments().

THis is because the Exynos u-boot, for example on the Chromebooks, doesn't set
up the CNTFRQ register as expected by arch_timer. Instead, we have to specify
the frequency in the device tree like this.

The attached change defers the initialization of the variables tsk, mm
and flags until they are needed. As a result, the code won't crash if a
kernel probe is done with a corrupt context and the code will be better
optimized.

parisc: fix interruption handler to respect pagefault_disable()
Running an "echo t > /proc/sysrq-trigger" crashes the parisc kernel. The
problem is, that in print_worker_info() we try to read the workqueue info via
the probe_kernel_read() functions which use pagefault_disable() to avoid
crashes like this:
probe_kernel_read(&pwq, &worker->current_pwq, sizeof(pwq));
probe_kernel_read(&wq, &pwq->wq, sizeof(wq));
probe_kernel_read(name, wq->name, sizeof(name) - 1);
The problem here is, that the first probe_kernel_read(&pwq) might return zero
in pwq and as such the following probe_kernel_reads() try to access contents of
the page zero which is read protected and generate a kernel segfault.
With this patch we fix the interruption handler to call parisc_terminate()
directly only if pagefault_disable() was not called (in which case
preempt_count()==0). Otherwise we hand over to the pagefault handler which
will try to look up the faulting address in the fixup tables.

parisc: kill SMP single function call interrupt
Commit
9a46ad6d6df3b54 "smp: make smp_call_function_many() use logic
similar to smp_call_function_single()" has unified the way to handle
single and multiple cross-CPU function calls. Now only one interrupt
is needed for architecture specific code to support generic SMP function
call interfaces, so kill the redundant single function call interrupt.

KVM: ARM: Add support for Cortex-A7
This patch adds support for running Cortex-A7 guests on Cortex-A7 hosts.
As Cortex-A7 is architecturally compatible with A15, this patch is largely just
generalising existing code. Areas where 'implementation defined' behaviour
is identical for A7 and A15 is moved to allow it to be used by both cores.
The check to ensure that coprocessor register tables are sorted correctly is
also moved in to 'common' code to avoid each new cpu doing its own check
(and possibly forgetting to do so!)

KVM: ARM: fix the size of TTBCR_{T0SZ,T1SZ} masks
The T{0,1}SZ fields of TTBCR are 3 bits wide when using the long descriptor
format. Likewise, the T0SZ field of the HTCR is 3-bits. KVM currently
defines TTBCR_T{0,1}SZ as 3, not 7.
The T0SZ mask is used to calculate the value for the HTCR, both to pick out
TTBCR.T0SZ and mask off the equivalent field in the HTCR during
read-modify-write. The incorrect mask size causes the (UNKNOWN) reset value
of HTCR.T0SZ to leak in to the calculated HTCR value. Linux will hang when
initializing KVM if HTCR's reset value has bit 2 set (sometimes the case on
A7/TC2)
Fixing T0SZ allows A7 cores to boot and T1SZ is also fixed for completeness.

KVM: ARM: Fix calculation of virtual CPU ID
KVM does not have a notion of multiple clusters for CPUs, just a linear
array of CPUs. When using a system with cores in more than one cluster, the
current method for calculating the virtual MPIDR will leak the (physical)
cluster information into the virtual MPIDR. One effect of this is that
Linux under KVM fails to boot multiple CPUs that aren't in the 0th cluster.
This patch does away with exposing the real MPIDR fields in favour of simply
using the virtual CPU number (but preserving the U bit, as before).

vfs: allow O_PATH file descriptors for fstatfs()
Olga reported that file descriptors opened with O_PATH do not work with
fstatfs(), found during further development of ksh93's thread support.
There is no reason to not allow O_PATH file descriptors here (fstatfs is
very much a path operation), so use "fdget_raw()". See commit
55815f70147d ("vfs: make O_PATH file descriptors usable for 'fstat()'")
for a very similar issue reported for fstat() by the same team.

ext4: fix memory leak in xattr
If we take the 2nd retry path in ext4_expand_extra_isize_ea, we
potentionally return from the function without having freed these
allocations. If we don't do the return, we over-write the previous
allocation pointers, so we leak either way.
Spotted with Coverity.
[ Fixed by tytso to set is and bs to NULL after freeing these
pointers, in case in the retry loop we later end up triggering an
error causing a jump to cleanup, at which point we could have a double

ARC: Ignore ptrace SETREGSET request for synthetic register "stop_pc"
ARCompact TRAP_S insn used for breakpoints, commits before exception is
taken (updating architectural PC). So ptregs->ret contains next-PC and
not the breakpoint PC itself. This is different from other restartable
exceptions such as TLB Miss where ptregs->ret has exact faulting PC.
gdb needs to know exact-PC hence ARC ptrace GETREGSET provides for
@stop_pc which returns ptregs->ret vs. EFA depending on the
situation.
However, writing stop_pc (SETREGSET request), which updates ptregs->ret
doesn't makes sense stop_pc doesn't always correspond to that reg as
described above.
This was not an issue so far since user_regs->ret / user_regs->stop_pc
had same value and both writing to ptregs->ret was OK, needless, but NOT
broken, hence not observed.
With gdb "jump", they diverge, and user_regs->ret updating ptregs is
overwritten immediately with stop_pc, which this patch fixes.

ACPI / hotplug / PCI: Drop WARN_ON() from acpiphp_enumerate_slots()
The WARN_ON() in acpiphp_enumerate_slots() triggers unnecessarily for
devices whose bridges are going to be handled by native PCIe hotplug
(pciehp) and the simplest way to prevent that from happening is to
drop the WARN_ON().

And one pain in the ass revert, so we have VGA arbitration that when
implemented 4-5 years ago really hoped that GPUs could remove
themselves from arbitration completely once they had a kernel driver.
It seems Intel hw designers decided that was too nice a facility to
allow us to have so they removed it when they went on-die (so since
Ironlake at least). Now Alex Williamson added support for VGA
arbitration for newer GPUs however this now exposes itself to
userspace as requireing arbitration of GPU VGA regions and the X
server gets involved and disables things that it can't handle when VGA
access is possibly required around every operation.
So in order to not break userspace we just reverted things back to the
old known broken status so maybe we can try and design out way out.
Ville also had a patch to use stop machine for the two times Intel
needs to access VGA space, that might be acceptable with some rework,
but for now myself and Daniel agreed to just go back"

VFIO: vfio_iommu_type1: fix bug caused by break in nested loop
In vfio_iommu_type1.c there is a bug in vfio_dma_do_map, when checking
that pages are not already mapped. Since the check is being done in a
for loop nested within the main loop, breaking out of it does not create
the intended behavior. If the underlying IOMMU driver returns a non-NULL
value, this will be ignored and mapping the DMA range will be attempted
anyway, leading to unpredictable behavior.
This interracts badly with the ARM SMMU driver issue fixed in the patch
that was submitted with the title:
"[PATCH 2/2] ARM: SMMU: return NULL on error in arm_smmu_iova_to_phys"
Both fixes are required in order to use the vfio_iommu_type1 driver
with an ARM SMMU.
This patch refactors the function slightly, in order to also make this
kind of bug less likely.

ALSA: hda - Fix microphone for Sony VAIO Pro 13 (Haswell model)
The external mic showed up with a precense detect of "always present",
essentially disabling the internal mic. Therefore turn off presence
detection for this pin.
Note: The external mic seems not yet working, but an internal mic is
certainly better than no mic at all.

Adding drm/i915 into the vga arbiter chain means that X (in a piece of
well-meant paranoia) will do a get/put on the vga decoding around
_every_ accel call down into the ddx. Which results in some nice
performance disasters [1]. This really breaks userspace, by disabling
DRI for everyone, and stops OpenGL from working, this isn't limited
to just the i915 but both the integrated and discrete GPUs on
multi-gpu systems, in other words this causes untold worlds of pain,
Ville tried to come up with a Great Hack to fiddle the required VGA
I/O ops behind everyone's back using stop_machine, but that didn't
really work out [2]. Given that we're fairly late in the -rc stage for
such games let's just revert this all.
One thing we might want to keep is to delay the disabling of the vga
decoding until the fbdev emulation and the fbcon screen is set up. If
we kill vga mem decoding beforehand fbcon can end up with a white
square in the top-left corner it tried to save from the vga memory for
a seamless transition. And we have bug reports on older platforms
which seem to match these symptoms.
But again that's something to play around with in -next.

method but unfortunately got merged later than commit

(dma: add driver for R-Car HPB-DMAC), so that the HPB-DMAC driver retained the
old prototype which caused this warning:

The newly added parameter is used to override DMA slave address from 'struct
hpb_dmae_slave_config', so we have to add the 'slave_addr' field to 'struct
hpb_dmae_chan', conditionally assign it in set_slave() method, and return in
slave_addr() method.
