PCI: Do not run NVidia quirks related to MSI with MSI disabled
There is no reason to run NVidia-specific quirks related to HT MSI
mappings with MSI disabled via pci=nomsi, so make
__nv_msi_ht_cap_quirk() return immediately in that case.
This allows at least one machine to boot 100% of the time with
pci=nomsi (it still doesn't boot reliably without that).

PCI: MSI: Restore read_msi_msg_desc(); add get_cached_msi_msg_desc()

2ca1af9aa3285c6a5f103ed31ad09f7399fc65d7 "PCI: MSI: Remove
unsafe and unnecessary hardware access" changed read_msi_msg_desc() to
return the last MSI message written instead of reading it from the
device, since it may be called while the device is in a reduced
power state.
However, the pSeries platform code really does need to read messages
from the device, since they are initially written by firmware.
Therefore:
- Restore the previous behaviour of read_msi_msg_desc()
- Add new functions get_cached_msi_msg{,_desc}() which return the
last MSI message written
- Use the new functions where appropriate

PCI: export SMBIOS provided firmware instance and label to sysfs
This patch exports SMBIOS provided firmware instance and label of
onboard PCI devices to sysfs. New files are:
/sys/bus/pci/devices/.../label which contains the firmware name for
the device in question, and
/sys/bus/pci/devices/.../index which contains the firmware device type
instance for the given device.

PCI: Allow read/write access to sysfs I/O port resources
PCI sysfs resource files currently only allow mmap'ing. On x86 this
works fine for memory backed BARs, but doesn't work at all for I/O
port backed BARs. Add read/write to I/O port PCI sysfs resource
files to allow userspace access to these device regions.

x86/PCI: use host bridge _CRS info on ASRock ALiveSATA2-GLAN
This DMI quirk turns on "pci=use_crs" for the ALiveSATA2-GLAN because
amd_bus.c doesn't handle this system correctly.
The system has a single HyperTransport I/O chain, but has two PCI host
bridges to buses 00 and 80. amd_bus.c learns the MMIO range associated
with buses 00-ff and that this range is routed to the HT chain hosted at
node 0, link 0:
bus: [00, ff] on node 0 link 0
bus: 00 index 1 [mem 0x80000000-0xfcffffffff]
This includes the address space for both bus 00 and bus 80, and amd_bus.c
assumes it's all routed to bus 00.
We find device 80:01.0, which BIOS left in the middle of that space, but
we don't find a bridge from bus 00 to bus 80, so we conclude that 80:01.0
is unreachable from bus 00, and we move it from the original, working,
address to something outside the bus 00 aperture, which does not work:

The BIOS told us everything we need to know to handle this correctly,

This was a regression between 2.6.33 and 2.6.34. In 2.6.33, amd_bus.c
was used only when we found multiple HT chains.
3e3da00c01d050, which
enabled amd_bus.c even on systems with a single HT chain, caused this
failure.
This quirk was written by Graham. If we ever enable "pci=use_crs" for
machines from 2006 or earlir, this quirk should be removed.

PCI: remove unused HAVE_ARCH_PCI_SET_DMA_MAX_SEGMENT_{SIZE|BOUNDARY}
In 2.6.34, we transformed the PCI DMA API into the generic device
mode. The PCI DMA API is just the wrapper of the DMA API.
So we don't need HAVE_ARCH_PCI_SET_DMA_MAX_SEGMENT_SIZE or
HAVE_ARCH_PCI_SET_DMA_SEGMENT_BOUNDARY (which enable architectures to
have the own implementations). Both haven't been used anyway.

PCI: disable mmio during bar sizing
It is a known issue that mmio decoding shall be disabled while doing PCI
bar sizing. Host bridge and other devices (PCI PIC) shall be excluded for
certain platforms. This patch mainly comes from Mathew Willcox's
patch in http://kerneltrap.org/mailarchive/linux-kernel/2007/9/13/258969.
A new flag bit "mmio_alway_on" is added to pci_dev with the intention that
devices with their mmio decoding cannot be disabled during BAR sizing shall
have this bit set, preferrablly in their quirks.
Without this patch, Intel Moorestown platform graphics unit will be
corrupted during bar sizing activities.

PCI: MSI: Remove unsafe and unnecessary hardware access
During suspend on an SMP system, {read,write}_msi_msg_desc() may be
called to mask and unmask interrupts on a device that is already in a
reduced power state. At this point memory-mapped registers including
MSI-X tables are not accessible, and config space may not be fully
functional either.
While a device is in a reduced power state its interrupts are
effectively masked and its MSI(-X) state will be restored when it is
brought back to D0. Therefore these functions can simply read and
write msi_desc::msg for devices not in D0.
Further, read_msi_msg_desc() should only ever be used to update a
previously written message, so it can always read msi_desc::msg
and never needs to touch the hardware.

PCI: Default PCIe ASPM control to on and require !EMBEDDED to disable
The CONFIG_PCIEASPM option is confusing and potentially dangerous. ASPM is
a hardware mediated feature rather than one under direct OS control, and
even if the config option is disabled the system firmware may have turned
on ASPM on various bits of hardware. This can cause problems later -
various hardware that claims to support ASPM does a poor job of it and may
hang or cause other difficulties. The kernel is able to recognise this in
many cases and disable the ASPM functionality, but only if CONFIG_PCIEASPM
is enabled.
Given that in its default configuration this option will either leave the
hardware as it was originally or disable hardware functionality that may
cause problems, it should by default y. The only reason to disable it
ought to be to reduce code size, so make it dependent on CONFIG_EMBEDDED.

PCI: kernel oops on access to pci proc file while hot-removal
I encountered the problem that /proc/bus/pci/XX/YY is not removed even
after the corresponding device is hot-removed, if the file is still
being opened. In addtion, accessing this file in this situation causes
kernel panic (see below).
Becasue the pci_proc_detach_device() doesn't call remove_proc_entry()
if struct proc_dir_entry->count > 1, access to /proc/bus/pci/XX/YY
would refer to struct pci_dev that was already freed.
Though I don't know why the check for proc_dir_entry->count was added,
I don't think it is needed. Removing this check fixes the problem.

ACPI: Disable ASPM if the platform won't provide _OSC control for PCIe
The PCI SIG documentation for the _OSC OS/firmware handshaking interface
states:
"If the _OSC control method is absent from the scope of a host bridge
device, then the operating system must not enable or attempt to use any
features defined in this section for the hierarchy originated by the host
bridge."
The obvious interpretation of this is that the OS should not attempt to use
PCIe hotplug, PME or AER - however, the specification also notes that an
_OSC method is *required* for PCIe hierarchies, and experimental validation
with An Alternative OS indicates that it doesn't use any PCIe functionality
if the _OSC method is missing. That arguably means we shouldn't be using
MSI or extended config space, but right now our problems seem to be limited
to vendors being surprised when ASPM gets enabled on machines when other
OSs refuse to do so. So, for now, let's just disable ASPM if the _OSC
method doesn't exist or refuses to hand over PCIe capability control.

PCI hotplug: make sure child bridges are enabled at hotplug time
Found one PCIe Module with several bridges built-in where a "cold"
hotadd doesn't work.
If we end up reassigning bridge windows at hotadd time, and have to loop
through assigning new ranges, we won't end up enabling the child bridges
because the first assignment pass already tried to enable them, which
prevents __pci_bridge_assign_resource from updating the windows.
So try to move enabling of child bridges to the end, and only do it
once.

PCI hotplug: shpchp: Removed check for hotplug of display devices
Removed check to prevent hotplug of display devices within shpchp.
Originally this was thought to have been required within the PCI
Hotplug specification for some legacy devices. However there is
no such requirement in the most recent revision. The check prevents
hotplug of not only display devices but also computational GPUs
which require serviceability.

PCI: Don't enable aspm before drivers have had a chance to veto it
The aspm code will currently set the configured aspm policy before drivers
have had an opportunity to indicate that their hardware doesn't support it.
Unfortunately, putting some hardware in L0 or L1 can result in the hardware
no longer responding to any requests, even after aspm is disabled. It makes
more sense to leave aspm policy at the BIOS defaults at initial setup time,
reconfiguring it after pci_enable_device() is called. This allows the
driver to blacklist individual devices beforehand.

x86/PCI: Add option to not assign BAR's if not already assigned
The Linux kernel assigns BARs that a BIOS did not assign, most likely
to handle broken BIOSes that didn't enumerate the devices correctly.
On UV the BIOS purposely doesn't assign I/O BARs for certain devices/
drivers we know don't use them (examples, LSI SAS, Qlogic FC, ...).
We purposely don't assign these I/O BARs because I/O Space is a very
limited resource. There is only 64k of I/O Space, and in a PCIe
topology that space gets divided up into 4k chucks (this is due to
the fact that a pci-to-pci bridge's I/O decoder is aligned at 4k)...
Thus a system can have at most 16 cards with I/O BARs: (64k / 4k = 16)
SGI needs to scale to >16 devices with I/O BARs. So by not assigning
I/O BARs on devices we know don't use them, we can do that (iff the
kernel doesn't go and assign these BARs that the BIOS purposely didn't
assign).
This patch will not assign a resource to a device BAR if that BAR was
not assigned by the BIOS, and the kernel cmdline option 'pci=nobar'
was specified. This patch is closely modeled after the 'pci=norom'
option that currently exists in the tree.

MSI delivery from on-board ahci controller doesn't work on K8M800. At
this point, it's unclear whether the culprit is with the ahci
controller or the host bridge. Given the track record and considering
the rather minimal impact of MSI, disabling it seems reasonable.

PCI quirk: AMD 780: work around wrong vendor ID on APC bridge
In all AMD 780 family northbridges, the vendor ID of the internal
graphics PCI/PCI bridge reads not as AMD but as that of the mainboard
vendor, because the hardware actually returns the value of the subsystem
vendor ID (erratum 18).
We currently have additional quirk entries for Asus and Acer, but it is
likely that we will encounter more systems with other vendor IDs.
Since we do not know in advance all possible vendor IDs, a better way to
find the device is to declare the quirk on the host bridge, whose ID is
always correct, and use that device as a stepping stone to find the PCI/
PCI bridge, if present.

PCI: change device runtime PM settings for probe and remove
This patch (as1388) changes the way the PCI core handles runtime PM
settings when probing or unbinding drivers. Now the core will make
sure the device is enabled for runtime PM, with a usage count >= 1,
when a driver is probed. It does the same when calling a driver's
remove method.
If the driver wants to use runtime PM, all it has to do is call
pm_runtime_pu_noidle() near the end of its probe routine (to cancel
the core's usage increment) and pm_runtime_get_noresume() near the
start of its remove routine (to restore the usage count). It does not
need to mess around with setting the runtime state to enabled,
disabled, active, or suspended.
The patch updates e1000e and r8169, the only PCI drivers that already
use the existing runtime PM interface.

CRED: Fix __task_cred()'s lockdep check and banner comment
Fix __task_cred()'s lockdep check by removing the following validation
condition:
lockdep_tasklist_lock_is_held()
as commit_creds() does not take the tasklist_lock, and nor do most of the
functions that call it, so this check is pointless and it can prevent
detection of the RCU lock not being held if the tasklist_lock is held.
Instead, add the following validation condition:
task->exit_state >= 0
to permit the access if the target task is dead and therefore unable to change
its own credentials.
Fix __task_cred()'s comment to:
(1) discard the bit that says that the caller must prevent the target task
from being deleted. That shouldn't need saying.
(2) Add a comment indicating the result of __task_cred() should not be passed
directly to get_cred(), but rather than get_task_cred() should be used
instead.
Also put a note into the documentation to enforce this point there too.

CRED: Fix get_task_cred() and task_state() to not resurrect dead credentials
It's possible for get_task_cred() as it currently stands to 'corrupt' a set of
credentials by incrementing their usage count after their replacement by the
task being accessed.
What happens is that get_task_cred() can race with commit_creds():

However, since a tasks credentials are generally not changed very often, we can
reasonably make use of a loop involving reading the creds pointer and using
atomic_inc_not_zero() to attempt to increment it if it hasn't already hit zero.
If successful, we can safely return the credentials in the knowledge that, even
if the task we're accessing has released them, they haven't gone to the RCU
cleanup code.
We then change task_state() in procfs to use get_task_cred() rather than
calling get_cred() on the result of __task_cred(), as that suffers from the
same problem.
Without this change, a BUG_ON in __put_cred() or in put_cred_rcu() can be
tripped when it is noticed that the usage count is not zero as it ought to be,
for example:

ALSA: hda - Add a PC-beep workaround for ASUS P5-V
ASUS P5-V provides a SSID that unexpectedly matches with the value
compilant with Realtek's specification. Thus the driver interprets
it badly, resulting in non-working PC beep.
This patch adds a white-list for such a case; a white-list of known
devices with working PC beep.

ecryptfs: Bugfix for error related to ecryptfs_hash_buckets
The function ecryptfs_uid_hash wrongly assumes that the
second parameter to hash_long() is the number of hash
buckets instead of the number of hash bits.
This patch fixes that and renames the variable
ecryptfs_hash_buckets to ecryptfs_hash_bits to make it
clearer.

x86,kgdb: Fix hw breakpoint regression
HW breakpoints events stopped working correctly with kgdb
as a result of commit:
018cbffe6819f6f8db20a0a3acd9bab9bfd667e4
(Merge commit 'v2.6.33' into perf/core).
The regression occurred because the behavior changed for setting
NOTIFY_STOP as the return value to the die notifier if the breakpoint
was known to the HW breakpoint API. Because kgdb is using the HW
breakpoint API to register HW breakpoints slots, it must also now
implement the overflow_handler call back else kgdb does not get to see
the events from the die notifier.
The kgdb_ll_trap function will be changed to be general purpose code
which can allow an easy way to implement the hw_breakpoint API
overflow call back.

GFS2: Use kmalloc when possible for ->readdir()
If we don't need a huge amount of memory in ->readdir() then
we can use kmalloc rather than vmalloc to allocate it. This
should cut down on the greater overheads associated with
vmalloc for smaller directories.
We may be able to eliminate vmalloc entirely at some stage,
but this is easy to do right away.
Also using GFP_NOFS to avoid any issues wrt to deleting inodes
while under a glock, and suggestion from Linus to factor out
the alloc/dealloc.
I've given this a test with a variety of different sized
directories and it seems to work ok.

DEFDCDC{2,3} being tied high
Per the da850/omap-l138 Beta EVM SOM schematic, the
DEFDCDC2 and
DEFDCDC3 lines are tied high. This leads to a 3.3V IO and 1.2V CVDD
voltage.
Pass the right platform data to the TPS6507x driver so it can operate
on the
DEFDCDC{2,3}_HIGH register to read and change voltage levels.

In TPS6507x, depending on the status of
DEFDCDC{2,3} pin either
DEFDCDC{2,3}_LOW or
DEFDCDC{2,3}_HIGH register needs to be read or
programmed to change the output voltage.
The current driver assumes
DEFDCDC{2,3} pins are always tied low
and thus operates only on
DEFDCDC{2,3}_LOW register. This need
not always be the case (as is found on OMAP-L138 EVM).
Unfortunately, software cannot read the status of
DEFDCDC{2,3} pins.
So, this information is passed through platform data depending on
how the board is wired.

ALSA: hda - Fix pin-detection of Nvidia HDMI
The behavior of Nvidia HDMI codec regarding the pin-detection unsol events
is based on the old HD-audio spec, i.e. PD bit indicates only the update
and doesn't show the current state. Since the current code assumes the
new behavior, the pin-detection doesn't work relialby with these h/w.
This patch adds a flag for indicating the old spec, and fixes the issue
by checking the pin-detection explicitly for such hardware.

causes an oops.
Move the call to ddebug_remove_module() down into free_module(). In this
way it should be called from all error paths. Currently, we are missing
the remove if the module init routine fails.

This fixes an issue triggered by running concurrent syncs. One of the syncs
would go through while the other would just hang indefinitely. In any case, we
never actually want to wake a single waiter, so the *_all functions should
be used.

[S390] etr: fix clock synchronization race
The etr events switch-to-local and sync-check disable the synchronous clock
and schedule a work queue that tries to get the clock back into sync.
If another switch-to-local or sync-check event occurs while the work queue
function etr_work_fn still runs the eacr.es bit and the clock_sync_word can
become inconsistent because check_sync_clock only uses the clock_sync_word
to determine if the clock is in sync or not. The second pass of the
etr_work_fn will reset the eacr.es bit but will leave the clock_sync_word
intact. Fix this race by moving the reset of the eacr.es bit into the
switch-to-local and sync-check functions and by checking the eacr.es bit
as well to decide if the clock needs to be synced.

[S390] Fix IRQ tracing in case of PER
In case user space is single stepped (PER) the program check handler
claims too early that IRQs are enabled on the return path.
Subsequent checks will notice that the IRQ mask in the PSW and
what lockdep thinks the IRQ mask should be do not correlate and
therefore will print a warning to the console and disable lockdep.
Fix this by doing all the work within the correct context.

perf, powerpc: Use perf_sample_data_init() for the FSL code
We should use perf_sample_data_init() to initialize struct
perf_sample_data. As explained in the description of commit
dc1d628a
("perf: Provide generic perf_sample_data initialization"), it is
possible for userspace to get the kernel to dereference data.raw,
so if it is not initialized, that means that unprivileged userspace
can possibly oops the kernel. Using perf_sample_data_init makes sure
it gets initialized to NULL.
This conversion should have been included in commit
dc1d628a, but it
got missed.

The Pstate transition latency check was added for broken F10h BIOSen
which wrongly contain a value of 0 for transition and bus master
latency. Fam11h and later, however, (will) have similar transition
latency so extend that behavior for them too.

[CPUFREQ] Fix PCC driver error path
The PCC cpufreq driver unmaps the mailbox address range if any CPUs fail to
initialise, but doesn't do anything to remove the registered CPUs from the
cpufreq core resulting in failures further down the line. We're better off
simply returning a failure - the cpufreq core will unregister us cleanly if
we end up with no successfully registered CPUs. Tidy up the failure path
and also add a sanity check to ensure that the firmware gives us a realistic
frequency - the core deals badly with that being set to 0.

[CPUFREQ] pcc driver should check for pcch method before calling _OSC
The pcc specification documents an _OSC method that's incompatible with the
one defined as part of the ACPI spec. This shouldn't be a problem as both
are supposed to be guarded with a UUID. Unfortunately approximately nobody
(including HP, who wrote this spec) properly check the UUID on entry to the
_OSC call. Right now this could result in surprising behaviour if the pcc
driver performs an _OSC call on a machine that doesn't implement the pcc
specification. Check whether the PCCH method exists first in order to reduce
this probability.

sysfs: Don't allow the creation of symlinks we can't remove
Recently my tagged sysfs support revealed a flaw in the device core
that a few rare drivers are running into such that we don't always put
network devices in a class subdirectory named net/.
Since we are not creating the class directory the network devices wind
up in a non-tagged directory, but the symlinks to the network devices
from /sys/class/net are in a tagged directory. All of which works
until we go to remove or rename the symlink. When we remove or rename
a symlink we look in the namespace of the target of the symlink.
Since the target of the symlink is in a non-tagged sysfs directory we
don't have a namespace to look in, and we fail to remove the symlink.
Detect this problem up front and simply don't create symlinks we won't
be able to remove later. This prevents symlink leakage and fails in
a much clearer and more understandable way.

Commit
0ede76fcec5415ef82a423a95120286895822e2d, "USB: remove uses of
URB_NO_SETUP_DMA_MAP" introduced a regression by inadvertantly removing
initialization of the transfer flags. This caused initialization
failures in the ums-karma driver. Fix the regression by zeroing it.
While at it, as Alan Stern points out, the initializers for
actual_length and status are handled by the core and error_count
only matters for isochronous urbs, so they don't need to be set here.
Remove them.

USB: xhci: Set Mult field in endpoint context correctly.
The bmAttributes field of the SuperSpeed Endpoint Companion Descriptor has
different meanings, depending on the endpoint type. If the endpoint is
isochronous, the bmAttributes field is the maximum number of packets
within a service interval that this endpoint supports. If the endpoint is
bulk, it's the number of stream IDs this endpoint supports.
Only set the Mult field of the xHCI endpoint context using the
bmAttributes field if the endpoint is isochronous, and the device is a
SuperSpeed device.

USB: adds Artisman USB dongle to list of quirky devices
When an attempt is made to read the interface strings of the Artisman
Watchdog USB dongle (idVendor:idProduct 04b4:0526) an error is written
to the dmesg log (uhci_result_common: failed with status 440000) and the
dongle resets itself, resulting in a disconnect/reconnect loop.
Adding the dongle to the list of devices in quirks.c, with the same
quirk Alan Stern's previous patch for the Saitek Cyborg Gold 3D
joystick, stops the device from resetting and allows it to be used with
no problems.

USB: xhci: Set EP0 dequeue ptr after reset of configured device.
When a configured device is reset, the control endpoint's ring is reused.
If control transfers to the device were issued before the device is reset,
the dequeue pointer will be somewhere in the middle of the ring. If the
device is then issued an address with the set address command, the xHCI
driver must provide a valid input context for control endpoint zero.
The original code would give the hardware the original input context,
which had a dequeue pointer set to the top of the ring. This would cause
the host to re-execute any control transfers until it reached the ring's
enqueue pointer. When issuing a set address command for a device that has
just been configured and then reset, use the control endpoint's enqueue
pointer as the hardware's dequeue pointer.
Assumption: All control transfers will be completed or cancelled before
the set address command is issued to the device. If there are any
outstanding control transfers, this code will not work.

USB: Fix USB3.0 Port Speed Downgrade after port reset
Without this fix, a USB 3.0 port is downgraded to full speed after a port
reset of a configured device. The USB 3.0 terminations will be disabled
permanently, and USB 3.0 devices will always enumerate as full speed
devices, until the host controller is unplugged (if it is an ExpressCard)
or the computer is rebooted.
Fajun Chen traced this traced the speed downgrade issue to the port reset
and the interpretation of port status in USB hub driver code. The hub
code was not testing for the port being a SuperSpeed port, and it fell
through to the else case of Full Speed.
The following patch adds SuperSpeed mapping from the port status, and
fixes the speed downgrade issue.

USB: xHCI: Fix another bug in link TRB activation change.
Commit
6c12db90f19727c76990e7f4801c67a148b30111 also seems to have
introduced a bug that is triggered when the command ring is about to wrap.
The inc_enq() function will not have moved the enqueue pointer past the
link TRB. It is supposed to be moved past the link TRB in prepare_ring(),
which should be called before a TD is enqueued. However, the
queue_command() function never calls the prepare_ring() function because
prepare_ring() is only supposed to be used for endpoint rings. That means
the enqueue pointer will not be moved past the link TRB, and will get
overwritten.
The fix is to make queue_command() call prepare_ring() with a fake
endpoint status (set to running). Then the enqueue pointer will get moved
past the link TRB.

USB: Expose vendor-specific ACM channel on Nokia 5230
Nokia S60 phones expose two ACM channels. The first is
a modem, the second is 'vendor-specific' but is treated
as a serial device at the S60 end, so we want to expose
it on Linux too.
