Add support for such modes in shadow page code. For such modes PT_GUEST_DIRTY_MASK, PT_GUEST_ACCESSED_MASK, PT_GUEST_DIRTY_SHIFT and PT_GUEST_ACCESSED_SHIFT should be set to zero.
